Actually your girlfriend can check all these issues at her local Civil Registration office without any problems… And for further process u should consult your attorney. Remember!!! Russian Embassy doesn’t help with such kind of issues!!!

It’s an official paper! If u want to give your baby your last name u have to visit Russia and be there at this moment(moment of registration). If u can’t visit Russia u can send a kind of Power of attorney…but it will be too long process. Otherwise there will be a gap in papers where they should place name of a Father or instead of Fathers last name will be Your girlfriend last name( first name she can put whatever she wants).
But u have to bring with u not only your passport, but also official translation(with Apostille stamp) of your papers(look at the list below…I guess some papers will be the same). Here is my advise… I know that she have a right to register baby in one month or more(she have to visit her local Civil Registration office and ask for this as an exception…).
Well, as I see finally u wanna to marry her, if u gonna do it in Russia( cause it will make process off getting US visa more easy for her, and the baby will be your advantage!… Here is what we need according to Russian law.( I compared it with http://travel.state.gov/law/citizenship/citizenship_758.html)
But here u need more documents:
1) AN OFFICIAL TRANSLATION OF THE INFORMATION PAGE OF YOUR PASSPORT(certified by notary) – u can make it in Russia.
2) Affidavit forms in which u state your current marital status. Certified copies of US documents must also bear an Apostille stamp to be valid in Russia – these stamps are only available in the US. ( u can translate and certified it also in Russia)
3) Permission for marriage with the citizen of foreign state (must also bear an Apostille stamp to be valid in Russia)
4)Copy of visa with the stamp of Russian local Passport and Visa department(u can do it in Rssia)
5) u have to wait one month since the moment when u fill all the papers in Civil Registration office, it means that u have to be in Russia for one month, but there r some exceptions – for example if u will bring a letter from your company that your vacations is only at this period only once in an year. they can reduce the time of waiting but it will be about 2 weeks.

U should check all this at your attorney.
Later u can change all, but u will place yourself in very tough situation. Try to find time to visit Russia in a month after baby birth…but bring all papers. Good luck!!!
